*Early Registration: $399.00 per person, deadline: On or before May 2, 2014.
General Registration: $420.00 per person: On or after May 3, 2014 through May 30, 2014.
Class size is limited to 38 attendees.
Certificates of attendance will be provided upon completion of course. This course has been approved by the Louisiana Board of Physical Therapy Examiners and by the Louisiana State Board of Medical Examiners for Occupational Therapy for 14.0 hours contact hours/1.4 CEU’s.

**Cancellation Policy**: We understand things happen. If you cancel 30 days or more prior to the course we will refund 100% of your tuition. Inside of 30 days here is what we will do:
| 21-29 days | 80% refund |
| 14-20 days | 70% refund |
| 7-13 days | 60% refund |
| 1-6 days | 50% refund |
*We will also give you the option of a credit to another Functional Shoulder course in the future if you so choose.
